Don Budge Scholarship Application Forms
Go to the last page of this site and print the page. Then fill out and submit form by Sept. 30 of the year after the school year ends (on high school graduation day). If the completed form and essay are submitted by email, the hand-written essay must be scanned and sent as a .jpg file attachment saved in a format not less than 150 dpi resolution. If this is not possible, contact the committee by mail.
Please send forms as a Microsoft Word document, in plain ASCII text, or a format that is compatible with PC (rather than Mac). If this is not possible, send forms by regular mail.
For physical copies of the form, please request with your address included.
